Request Assistance

To request transportation assistance from Angel Flight, please do one of the following:

• Complete an on-line mission request. A mission screener will contact you after your request is recieved. Click on:  http://afids.angelflightcentral.org/mission_request.asp

• Download and review the Assistance Request Packet, complete and fax the Flight Request and Medical Release Forms included in this packet to 816-421-2409. A mission screener will review your request and contact you as soon as possible upon receipt.
